The New York City Community Health Survey (CHS) is a telephone survey conducted by the Department of Mental Health and Hygiene (DOHMH).

DOHMH uses the survey results to address health problems and improve services for residents in NYC.

The survey uses a sample of randomly selected adults age 18 and older from all five boroughs.

Survey participants will be asked questions about their:

  • Health
  • Health practices
  • Access to health care
